The Parkinson Network of Mt. Diablo’s (“PNMD”) mission is to provide information, resources, and social interaction to support people living with Parkinson’s Disease (“PD”) and their caregiving community.
Managing PD well requires a team effort. Caregivers are an integral part of any PD team, but some persons with Parkinson’s (PwP) must deal with PD alone (“Fly Solo”). Through our new Flying Solo grant program PNMD will provide support to PwP’s who are on their own through a self-care grant program, that encourages them to add occasional assistance from outside resources.
1. Provide the Flying Solo PwP with periodic assistance with activities of daily living (ADLs), transportation, companionship, light housekeeping, personal care, etc.
2. Create a coherent care plan of outside services,
3. Provide financial assistance to help member households pay for professional, reliable, and scheduled care through an outside agency or service provider.
Grants are awarded in an amount not to exceed $1200.00 per member household per year. This amount is allocated for direct services from a licensed “service provider.” Services include an initial site visit, the development of a care plan, and the delivery of subsequent home care services. All administrative costs are absorbed by PNMD. There is no cost to the applicant.
